A fascinating new study published in Nature Medici A fascinating new study published in Nature Medicine analyzed blood samples from more than 60,000 people and estimated the biological age of over 40 different cell and tissue types.

The findings challenge the idea that we age at the same rate throughout the body.

Your brain may be aging differently than your muscles. Your immune system may be on a completely different timeline than your liver.

Perhaps even more compelling, the researchers found that the fastest-aging tissues were associated with future disease risk over 15 years of follow-up.

One finding stood out to me:
Muscle biological age was one of the strongest predictors of mortality, even beyond genetic risk.

While this research is observational and doesn't prove that reversing a tissue's biological age will change outcomes, it reinforces something we've been talking about for years:

Healthy aging isn't about one organ, one biomarker, or one intervention.

It's about supporting the systems that allow us to remain resilient over time.

For me, that includes prioritizing:
✔ Resistance training
✔ Adequate protein
✔ Metabolic health
✔ Sleep
✔ Nutrition
✔ Stress resilience

The science of biological aging is evolving quickly, and studies like this are helping us better understand that aging isn't a single process—it's a story unfolding across the entire body.

What do you think we'll be measuring routinely in the next decade: biological age, organ age, or something even more personalized?

We often think of sleep challenges as a stress pro We often think of sleep challenges as a stress problem, a hormone problem, or simply a consequence of modern life. And while all of those factors matter, there's another piece of the puzzle that's receiving increasing scientific attention: cellular energy.

Women are significantly more likely than men to experience difficulty falling or staying asleep. So when I came across a new study examining the relationship between creatine and sleep in women, it caught my attention.

In this randomized, double-blind, placebo-controlled trial, researchers followed women participating in a supervised resistance training program for six weeks. Those taking 5 grams of creatine monohydrate daily slept an average of 48 minutes longer on training nights than those taking a placebo.

Why might this happen?

Intense exercise places enormous demands on our cells' energy systems. As ATP is depleted, the body relies on phosphocreatine (PCr) to rapidly regenerate energy. When those reserves become exhausted, energy-sensing pathways, including AMPK, are activated, signaling that the cell is under metabolic stress. That lingering "tired but wired" feeling many of us recognize after a demanding day or workout may, at least in part, reflect this energy imbalance.

By helping replenish phosphocreatine stores, creatine may support more efficient energy recovery after exercise, potentially creating the conditions for deeper, longer sleep.

This is an early study with a relatively small sample size, so we shouldn't overstate the findings. But it's another fascinating example of how supporting cellular physiology may have benefits that extend well beyond muscle performance.

Sleep, after all, begins at the cellular level.

I'd love to hear your thoughts. Have you noticed changes in your sleep after adding creatine to your routine?

Even with lifestyle interventions and GLP-1 medica Even with lifestyle interventions and GLP-1 medications, many patients continue to experience significant post-meal metabolic stress, a key driver of insulin resistance, prediabetes, visceral adiposity, and cardiometabolic disease.

In this webinar, I join Dr. Sanjay Bhojraj and Dr. Eric Johnston from @sigrid.pro to explore an emerging approach that targets this often-overlooked "metabolic gap" at the meal itself.

During the session, we discuss:
✔ Why post-meal glycemic and lipemic spikes matter for long-term metabolic health
✔ The gut-localized mechanism behind SiPore® enzyme entrapment technology
✔ Clinical outcomes from the SHINE trial, including changes in HbA1c, visceral fat, LDL-C, and lean mass preservation
✔ Practical protocols for integrating SiPore® Capsules & Liquid into care for prediabetes, weight management, and patients transitioning on or off GLP-1 therapies

If you're looking to expand your clinical toolbox with evidence-based strategies for addressing postprandial metabolic dysfunction, I think you'll find this discussion valuable.

For decades, gut permeability has largely been fra For decades, gut permeability has largely been framed as a tight junction problem.

This paper suggests that framework may be incomplete.

Emerging evidence points to another layer of gut barrier biology involving goblet cells, the microbiome, dietary PUFAs, and leukotriene signaling that may influence how dietary antigens are sampled by the immune system.

If these mechanisms continue to hold up, they could expand how we think about gut barrier integrity, immune tolerance, and even nutrition-based interventions.
This isn't about replacing what we know about tight junctions.

It's about recognizing that the gut barrier appears to be far more dynamic than a single epithelial layer.

As functional medicine clinicians, this is exactly the kind of science worth paying attention to. The more we understand the biology, the more precise our interventions can become.

I break down the paper, the proposed mechanism, and what it could mean for clinical practice in my latest article.

💬 Comment PUFA and I'll send you the full article exploring the emerging science behind the gut PUFA-microbiome axis and what it may mean for clinical practice.

In my practice, I consider hormone replacement the In my practice, I consider hormone replacement therapy for many women when it's appropriate and can be prescribed safely. In fact, it's a therapy I've chosen for myself.

That doesn't mean HRT is right for everyone. Like any medical intervention, the decision should be individualized, weighing potential benefits and risks based on your unique health history and goals.
